For proof you do need to show you own the 100 shares of stock. You can provide either a copy of the proxy form (you get this every year with the annual report) or a copy of the statement from your broker or another broker form that would show you own the stock. Most suggest you white-out your account number. You also need to send them your booking number and I usually will send a copy of the invoice from the TA. This all can be sent by fax or snail mail. The address and info are in the annual report. Since I am out of the country at the moment I do not have the detailed info to post. The credit has always showed-up on our on-board statement (sometimes you do not see the credit for a few days) and its simply netted against any on-board charges against your account.
