Just came back from RCL Grandeur of the Seas (2/23 to 2/28). Arranged through RCL to do the aerial trek and cave tubing (paid $135 each). It was horrible! We spent more time waiting than doing the actual fun stuff. We started 6:45 in the morning and got back to the ship around 3:15pm. The actual time spent on the aerial trek was roughly 3 minutes; and the time spent on the cave tubing was approximately 15 minutes.
Sorry you had such a bad time. I went cave tubing only with a non-cruise arranged excursion. Had a rocky start because the guy had us booked for the following year! But, once everything was ironed out we had a blast! He took us thru an extra cave because of the mix up and we spent a very relaxing enjoyable day tubing thru the caves. He pointed out the cruise people on their excursion and where they started and stopped and you guys get taken! Sure glad I did it with him and not the cruise line.

TammyJo is my sister, so I can speak for her as she doesn't get on the forums often. WE used ReggieTours, you can find them on the internet.
Many people on this forum recommend him and he didn't disappoint. Although recently someone did have a mix up that has not been resolved yet, everyone else I know that have toured through Reggie had a good experience. It is much less expensive than booking through the ship and they give you a longer tour than the ship.
I normally only book with tour guides that have been recommended by others. You have already done that. You can look through this forum for more names if you want. Reggie and Xtreme Cave tubing are the most used.
Also, make sure you will be refunded any deposit should the ship not port for some reason. All the reputable business will state this. Don't book with any guide that won't. Cruise itineraries get changed for many reasons (weather, mechanical troubles, etc) and the tour operator's in the business know this and won't hold it against you.
I'm not sure there is much more you can do. Unfortunately, there is not such thing as an international better business bureau or something you can check with. That is why the word of mouth thing is so useful.
We have used Xtreme or Extreme Tours for Zipline and Cave tubing and we had a great time. We did have to wait about 10 minutes for the zipline but we had a lot of time on it and cave tubing too. We had a group of 8 people and our guide JP was smart, funny and so helpful...we even got great tshirts too
