I know that you can use your birth certificate as identification in Jamica, but what about the Cayman Islands? I didn't think about a passport earlier, and now time is $$$$. If I can get away with using my birth certificate there, it would be great.

Yes, a certified (one with a raised seal) copy of your birth certificate is fine. Actually, the cruise line will verify that you have it when you check in for the cruise. It won't be necessary to produce it when you go ashore.
If you plan on doing any travel in the future, I would go ahead and get a passport, as it makes it so much easier.

When you get off the ship at each port you will only be required to carry your sail and sign card and a photo ID. Make sure in addition to your official birth certificate you have your driver's license or other form of gov't photo ID.
