I'm assuming you're interested in Carnival since you posted in this category. BUT I did want to tell you that the gratuity suggestions for Norwegian are as follows, quoted from Vacationstogo.com:
"For passengers over the age of 12: $10.00 per adult, per day automatically charged to onboard account. For children age 3-12: $5 per child, per day. For children under the age of 3: no charge."

Although the specific question has not been asked, I do wish to say that children use the services of the crew every bit as much as adults - at least when it comes to those who get part of the tip charges. The waiters for instance really cater to the children.
And the childern use the Camp Carnival staff whihc may not be included in that mandatory tip, so if you make use of Camp Carnival, consider giving them something too..
And while you are "Auto-matically" charged those tips, be aware that if the service does not warrant it, you may have them adjusted.

This link breaks down the tips. We normally tip our cabin stewart, and Waiter and helper a little extra. What is considered normal on this do most people tip extra and how much?
On tipping over and above, just go with "your gut" because anything over the fixed amount is certainly appreciated. I always tip the cabin steward approx $20 on the first day and the wait staff on the last evening, about the same percentage - 1/2 of their fixed breakdown amounts. Perhaps a 5 to the maitre'd, more or less depending on his involvement. If everyone gave him 5 bucks, he'd be the highest paid on the ship! But, tipping more is only justified with actual performance. Put yourself in the shoes of the person to be tipped. If you would expect more, they probably would too.

The tips for the maitre'd still annoy me. Some lines expect you to give them something. Of course, all they do for most people is come around every night to glad-hand you and make sure you see them. Or they may serve one or two plates in some attempt to appear helpful. Of course the big ploy is when they appear on lobster night to remove the shell from everyone's lobster.
