West Mediterranean Cruise Ports

We will be cruising in October (Brilliance of the Seas). My husband wants to book everything ahead through Royal Caribbean. He's afraid tours will be filled. I want to maybe book a few tours ahead, but wait on others. We have already visited popular areas as Rome/Florence/Amalfi Coast in the past. So we could tour other places which may not fill as fast. Also does the ship dock close enough to any of the ports so we could walk around ourselves? Thanks for any help you can give us. )
