If we take a cab...does it matter that the 2 year old is not in a car seat? How does this work?

Are you talking about a cab here in America? I think it is law in every state now that a child must be in a car seat.
if it is in another country I dont know what the laws are but if you are going to be in cozumel I would make sure the child is in a safety seat.the taxi drivers in cozumel drive as if traffic laws are only a suggestion.

We would take a car seat when we took our son on cruises when he was little. We used it on the airplanes and in cabs. If the cab didn't have seat belts readily accessible we wouldn't use them.

Bring the car seat along on the ship. It won't hurt, especially since you are driving to Miami. No airline hassles to deal with.

Although cabs are not prevalent in Key West car seats are legally required should you use a cab or any car there. Obviously you won't need it at Coco Cay since there aren't any cars there. I'd use it in Nassau if I took a taxi.

Right Nassau is the only port we would need it for...worried about the room size 3 people in 122 sq ft room...with "crib" and now the car seat?

I wonder if cabs carry car seats in their trunk or maybe you could let them know you have a small child that would need a car seat, ahead of time and they can bring a car seat. Or you could see if the child's "other" Grandma could assist in driving to the port and taking the car seat back home. I'm sure she wouldn't mind. As far as driving around Nassau...Can't help you there.
