What if a body builder or some huge muscle mass person were unable to fit in the seat, how would they handle that situation?
I have the same question. I was on a flight recently during which the gentleman sitting next to me was slender, yet had very broad shoulders that were wider than his seatback. I had to sit tilted to my left for the whole flight. It's also not the first time it's happened.
Oh... and don't get me started on the carry-on rule.
If they make you buy two seats then shouldn't you get two of whatever goes with a seat, like double luggage allowance, double carry-ons, etc.? So if the first bag is $25 and the second is $50, couldn't you say you have just one bag for each seat and save $25?
This article describes the policy in Canada. Basically, they consider obesity a disability, and with a doctor's note, which is good for two years, the airlines are obliged to give someone a second seat if the passenger can't fit into just one seat.

Absolutely, Dave!! And I would push that point to the limit. If the flight is full and they try to squeeze someone into the seat that I had to pay extra for, it's not going to be a pleasant sight. If I am paying for it, woe be to the person who tries to infringe on my space.
Try being tall, 6'2 1/2", heavy 300 lbs., wide shoulders with big arms because I lift weights and size 15 shoes. Now try to be comfortable. The floor space also gets smaller with large feet. I can always buckle the seat belt.
