I have never done the captain's dinner. Is this formal or is there something else that is formal that we can attend. What is considered formal. Formal like prom wear, or dress slacks, and blazer coat? do we have to pre-reserve this ?

You will see a wide variety of dress on formal night. Short Bahama cruises tend to be less formal simply because many people don't want to pack the formal clothes for such a short cruise. However; you may see a variety of all the things you mentioned. Pre-reserve? Don't quite understand, if you mean prereserve the dinner-No.
