This is a great packing list.
As for what's included and what isn't: generally spoken, most drinks / beverages aren't. These include soft drinks, beer, wine, cocktails etc.
On the other hand most of the food is included, this includes food served in the main dining room(s), buffet restaurant and pool grill (if your ship has a Johnny Rockets - that one's not included), but also room service. If your ship has an alternative restaurant or two, those incur an extra charge. Depending on the ship you will be sailing on, there might be some cafés etc. which offer not included items, as well.
Most of the activities onboard are included, as well, with the obvious exceptions (casino, bingo, art auctions etc.).
Be warned - cruising is addictive!
