I have printed off my setsail pass. Can anyone tell me if it is only the lead name that has to sign the forms as my husband's form has no where on it to sign. Thank you for your help.I am sailing on IOS in June 2009.

You have two Setsail passes, one for you and one for your husband? It is usually just one page with the need for only one signature - that being the first name on the booking.

We have 4 people in our stateroom. My name is the one on the credit card and my daughter is using her credit card for her expenses. So when I printed our setsail pass, there were 3 sheets; one for my husband & son without a signature, one for my daugher with her credit card info, and one for me with my credit card info. the only ones that needed signatures were my daughter's and mine. I'm assuming the signatures have to do with credit card but that is an assumption based on how the setsail printed out.

Thanks for the clarification. Since we've always just used one card we only had one page for the SetSail pass.

The signatures are for the credit cards. Look closely and you should see two places for a signature relating to the credit card. They want it signed in both places.
